This app allows families to easily track dementia-related issues and coordinate care between family members and paid caregivers while providing evidence-based knowledge to best navigate the many decisions that need to be made. By guiding families through our complex, and often broken, healthcare system, Alzheimer's Care Partner helps families provide the highest quality of care possible for their loved ones. This app currently supports families who are caring for someone living with Alzheimer's Disease, Vascular Dementia, Lewy Body Dementia (LBD), Frontotemporal Dementia (FTD), Parkinson's Disease Dementia, and 6 additional types of dementia.

For the paid plans you will be charged to your Apple Account, which will automatically renew unless canceled at least 24 hours before the end of the current period. Auto-renew can be turned off anytime by going to your Account Settings after purchase. No cancellation of the current subscription is allowed during the active subscription period.
